		<description>This is such an important point- yes, we are being encouraged to &quot;share decision making&quot; as patients, but we are not given all the information we need to be at the centre of our own care, or to share that knowledge with all the healthcare professionals involved , because the IT systems dont speak. There are also many people on both sides who need more training in sharing decision making to gain the skills required.

When I was pregnant I was given my file to carry around, and could  decide who I would share it with. I also wrote my own birth plan which was adhered to, so I did feel in control, and the whole experience was a very positive one- well done St George&#039;s!

It would be great to promote this attitude again, so those of us with long term conditions were acknowledged to be at the centre of our own care.

Lets invite some others to join in this conversation!</description>
